Eastman provides a choice of two dental plans to you through BlueCross BlueShield of Tennessee (BCBST) – EastDent 1 and EastDent 2. Both of these plans cover preventive, restorative and orthodontia services. Payment for these benefits is through payroll deductions and eligibility follows the Eastman eligibility rules as set forth in our plan documents.

  1. Preventive Services – Include routine exams and cleanings, X-rays. Fluoride treatments for dependent children under age 19.
  2. Restorative Services – Include fillings, extractions, oral surgery, periodontics, crowns, dentures, and bridgework.
  3. Annual Limit – Applies to all services except orthodontia and implants.
  4. Orthodontia – Includes braces, retainers, and visits to the orthodontist for your dependent children under age 19.

* For in-network services, the member will NOT be held liable for balances above the negotiated amount allowed by the dental third-party administrator.

NOTE: For out-of-network services, plan reimbursements are based on an allowable expense which is the maximum dental charge considered reasonable and customary for a particular service in a particular geographic area. You are responsible for any amount that may be billed by the provider that is above reasonable and customary.

Eastman offers our employees and their eligible dependents vision insurance through BlueCross BlueShield of Tennessee. Our vision plan covers routine eye exams at 100% and also provides coverage for eyeglasses, contact lenses, and laser eye surgery.

Just when you think you have it figured out, along comes a challenge. Whether those challenges are big or small, your Cigna EAP is available to help you and the members of your household find a solution and restore your peace of mind.

The EAP is a confidential resource that can help you and the members of your household identify and resolve problems 24/7.

EAP representatives can help you find counseling services or direct you to a variety of resources in your community and online. Up to three face-to-face sessions with a specialist in your area are available at no cost to you. EAP also offers free telephone wellness seminars.

Optional Life Insurance

You can purchase additional life insurance of 1 to 7 x IASR rounded to the nearest $100. Your election maximum is subject to the lesser of a combined maximum of $3,000,000 for basic and optional coverage or 8 x IASR. Your life insurance is payable to your designated beneficiary.

If you elect to increase your current coverage amount more than 1 x IASR or change from no coverage to coverage during open enrollment, you must provide POGH even if you were previously eligible for this benefit.

Example: Your optional coverage in 2016 was 1 x IASR. You have elected to increase to 3 x IASR. Since you have increased more than one level, POGH is required.

You can purchase optional life insurance coverage for your spouse at the following levels:

Accidental Death & Dismemberment Insurance (ADI)

ADI provides for your beneficiary if you die as the result of an accident or provides for you if you suffer an accident-related injury. You can purchase coverage at the following levels:

Spouse DADI provides for you if your spouse dies as the result of an accident or suffers an accident-related injury. You can purchase coverage at the following levels:

Occupational Accidental Death Insurance (OAD)

OAD is provided by Eastman to you at no cost. Your OAD coverage is 2 x IASR. OAD covers you for accidental death that occurs as a result of an accidental injury while you were performing customary duties at your normal place of business or at other places to which Eastman’s business requires you to travel.

Disability coverage protects you in the event you lose income due to a disabling sickness or injury for an extended period of time.

Short-Term Disability (STD)
Eastman provides Short-Term Disability for non-job related illness or injury for up to 1,040 hours. Should you have a work-related illness or injury, Eastman's Worker's Compensation Supplement Plan provides up to 1,040 hours of coverage.

After your STD benefits expire, Eastman’s Long-Term Disability Plan may provide a portion of your income as long as you continue to qualify under the provisions of the LTD plan.

Long-Term Disability (LTD)
Long-term disability coverage is an income replacement program that protects you and your family in the event you become disabled, have exhausted your short-term disability benefit and are unable to work. Eastman provides 50% coverage to you at no cost. If you choose to enroll in the 60% option, your biweekly contribution will be deducted from your pay.

Proof Of Good Health (POGH) is required when increasing your coverage from 50% to 60%. You will be asked to complete a brief questionnaire online.

Contributions for the long-term disability 60% coverage option will be deducted from payroll after taxes.
